Position: Transitions Facilitator
Reports to: Managing Director
Department: Family Support Team
Position Summary:
The Transitions Facilitator will assist people with a disability and their families in an individualized and person-centered way to plan and take action towards their vision for a good life.
This may include:
- building a vision,
- keeping people and their families informed of their options,
- supporting the development of short and long-term goals,
- offering guidance and advocacy on how to navigate various systems and supports,
- and to build community connections that foster valued roles that lead to belonging.
They may do this on their own, or in collaboration with other facilitators on our team. Tapping into their many connections to community, personal interests, ideas and creativity, the facilitator will support people to think creatively and to explore possibilities for building a good life, firmly rooted in community.
Guided by the organization’s values, that community is the starting place for inclusion and belonging, the facilitator will work collaboratively with each person, their family and support circles to help people to be proactive and prepared for various stages and phases of their life. A majority of the facilitator’s time will be working with youth, still attending school and transitioning out of the school system, to imagine a meaningful life beyond secondary school.
This entails:
- building a vision and identifying roles and pathways they would like to explore,
- ensuring essential applications for services and funding are completed,
- and to actively build connections in communities that will support people and their families to develop sustainable, reliable and relevant plans for what life will look like when they leave school.
In addition to direct work with families, a key to this role will be leveraging community capacity and leadership. In collaboration with the team, the facilitator will support the organization to keep a “finger on the pulse” of current and emerging needs of the people they support, seeking allies, collaborators and innovative opportunities and solutions for addressing challenges people with a disability and their families in Perth and Huron are facing as well as opportunities for innovation, creative partnerships and solutions.
Supporting events, developing resources and some community outreach work are all part of the scope of this role.
Responsibilities:
- Provide individualized support to each person and their family to guide them through the decisions and resources that will best suit them and their vision.
- Attend bi-weekly meetings and provide updates on planning outcomes
- Follow organization processes and maintain detailed records to track, report and log communication, plans and outcomes for each person supported.
- Work in collaboration with service providers, planning tables and other supportive individuals who assist with the development of a plan or a person’s care
- Stay up to date with relevant legislation, policies and processes regarding funding, applications, processes and procedures relevant to families and people with a disability
- Assist in the development of family and student-focused resources to assist youth and their families to dream and think creatively about building their future
- Plan and engage in conversations with community contacts to secure volunteer positions, CO-OP positions, employment or to find interest-based opportunities for people to explore
- Support some research and reporting
- Demonstrate a “all hands on deck”, positive, supportive and professional relationship with all staff, families, volunteers
- Maintain strict confidentiality regarding Facile Perth’s business, policies and procedures as well as people’s personal information
- Perform all duties and responsibilities in a manner consistent with Facile Perth’s Mission, Values, Vision and Principles of Support
Other duties may be assigned as required.
Skills and Qualifications:
- College or University education in the fields of Developmental Service Worker, Child and Youth Worker or Education, Community Development, or Social Work or a combination of some education and relevant experience.
- Experience working with or holding personal relationships with people with an intellectual or developmental disability or other disabilities, their families, or other marginalized groups is considered an asset.
- Strong motivation to improve the lives of people living with a disability and a belief that all people have the right to live with dignity, respect and feel a sense of belonging in the community where they live.
- Awareness of the developmental service sector, DSO, ODSP , Passport programs will be considered an asset but willing to provide training
- Strong written and verbal communication skills, excellent time management and ability to work independently to set deadlines and timelines
- Proven ability to make and maintain relationships with various people including (but not limited to) volunteers, community and educational partners, youth, and family members.
- Strong organizational and reporting skills are essential
- Excellent attention to detail
- Strong motivation to improve the lives of people living with a disability and a belief that all people have the right to live with dignity, respect and feel a sense of belonging in the community where they live.
- Valid Driver’s Licence and access to a vehicle.
- The Successful candidate will be asked to complete a VSS screening and will be required to purchase professional liability insurance at their own cost after an offer has been made.
Working Arrangements:
This is a contract position until August 2025 with the possibility for extension based on project funding. Hourly rate will be negotiated based on experience and education. This is an independent contractor position with flexible hours and the ability to work from home. It is an excellent opportunity for people who desire a flexible schedule, or who want to explore the field(s) of education or social services and receive relevant training on disability related programs, services and the inclusion movement.
Individuals will be required to travel within Perth and Huron County.
